Sub Acute Care
The Sub Acute Service at the Jewish Home at Rockleigh is designed to provide skilled nursing and rehabilitation services that will aid in the recovery process and return the individual to a level of functioning that will allow for a return to their home. Treatments are medically based and provided by highly skilled nursing and rehabilitation specialists. Residents in the Sub Acute service enjoy all of the services and amenities of the Home.
Because the resident is expected to return to their home or family member’s home, discharge planning begins immediately. The Social Service and Rehabilitation staff will assist in identifying post discharge needs and making appropriate referrals for home care and equipment.
Coverage for care is generally provided through Medicare Part A and supplemental insurance plans.
